Well, again, not looking good. One more thing to try, and if that doesn't work it's sort of over.
Try the clone again, same parameters, but this time (in DiskPatch 'Options'):
- enable disk reset after error

And, set readdelay to on.
ReadDelay is a hidden option. To get to it you must exit the DiskPatch start menu (the menu that appears first after starting from the DiskPatch start disk) by pressing Escape. You'll end up at the DOS prompt. Type "dp.exe /set" without the quotes and press Enter. You'll see a settings screen, the first setting is ReadDelay. Backspace over the 0 and type a 1, then press Enter through all settings until finished.
This change is persistent so restarting DiskPatch (to get to forensic mode) will keep readdelay enabled until you change it back to 0.

And, did you try the disk in another PC?

Good morning Tom,

Doesn't give you a 'warm feeling', does it?  I think the drive is dead.

When I bought my new large drive last week (the one we've been using as the clone target) I used the manufacturer's diskwizard to create a bootable clone of my old C: drive (the 40GB drive) and once that was done I took the small drive out and used the new drive as C:.  I have to say the difference in performance was amazing!  In effect, my PC then became 'another PC', didn't it?  Same chassis and operating system but from new hardware? That was where we started and when we wanted to use DP to clone the problem drive I put the old small drive back in as the boot device and wiped the new drive ready to receive the DP clone.  I've also tried the problem drive on other computers but via a USB enclosure which, I'm now aware from reading all your info, isn't good.

If you don't think the hardware change above counts as 'another PC' I do have another alternative.  However, as this other PC is built into some furniture it would not be a 5 minute task, but if you think it worth it let me know and I'll go ahead.

Meanwhile, the old 40GB drive is giving me cause for concern: the health check shows 70% performance and 97% health and it 'tinkles' when it runs, so if you agree we probably won't get any further with this computer I'd like to use the diskwizard to clone the boot drive again before I lose the operating system...  To do that, do I need to first format the drive so it expects NTFS (currently RAW)?

The resets fail too so the problem disk is pretty much toast, it seems. The idea of putting the disk in another PC came from the power surge thing; it would be nice to see how the disk behaves in hardware that is not subjected to such torture. Still, if USB didn't work either we can safely assume that this disk is gone.

We've seen odd disk behavior in the past being caused by faulty power supplies. I can't tell how much work it would be to switch the (surge suffering) power supply, but given where we are now I'm hesitant to give you more work. It's your call but we here think that this is pretty much over, we're not likely to get anything from this disk. There still are possibilities (like switching the control board of the disk) but you'd be getting into specialized territory there, and it will be costly. From here on, if more needs to be done with this disk, you'd have to seek out a local hardware recovery firm.

You may want to focus on that 40Gb disk, and your plan is solid. From the sound of it I'd do that ASAP.
You don't need to prepare the target disk in any way, remember that a clone is an exact image on a sector level, so every aspect of the source drive is copied. Filesystem types don't matter at that level. Your software may do things a little different than DP does but it should not be necessary to prepare the target disk in any way, though wiping the first few tracks might be wise. That way there's nothing on it that may throw off your clone software.

I decided the other PC would probably benefit from some maintenance so I'm doing it anyway...parts of computers everywhere now!  It does not get used too often so Windows decided it would install updates...once that's done I'll connect the problem disk and let you know what happens.

While using the diskwizard it asks:

The destination hard disc drive you have chosen contains some partitions that could contain useful data.  Click OK to confirm deletion of all the partitions on the destination hard disc drive.

I don't think the DP clone actually managed to read anything to copy it, so am I OK to go ahead?  The drive can't be read with Windows ("needs formatting") so I can't check.

Well, the MBR of the problem disk was read now and then, so if that was cloned the software you are using may think the target disk contains volumes: the partition table is in the MBR. This sort of thing is why I suggested the partial wipe. It's not a big deal though, you should simply let the program run and it should write over any data that is there.
So if you're sure about the source and target disk (getting them mixed up is obviously disastrous) just go ahead.

I tried the problem disk in the other computer and it didn't like it at all...booting took forever and once running the PC didn't see it either in Explorer or Disk Management.

The operating system has been recloned onto the new disk and the 40GB disk put to one side, just in case.

Tom, I think all that remains is to say a huge 'thank you' for all the help, advice and support the team there have provided, particularly the 'midnight watch'.  For anyone reading this saga the key lesson is to back up methodically and not get into this mess in the first place.
