I recently ran into an issue with installing XP SP2 that initially seemed like a disk problem. While it looks like the issue may be the service pack being confused because there I had an HDD attached with another XP partition, I ran the DiskPatch trial version surface scan and Support Log to see what it said. The surface scan reported no issues. At this time I am unable to interpret the support log. Can anyone tell me if it has found any issues?

Some odd things and a lot of junk (leftover partition tools stuff?) but no problems. Should work fine, so it's safe to assume that the problem you have is in the volume.
If you'd like to be sure the disk is healthy (it's an old disk) you can run this:
http://www.diydatarecovery.nl/dp_manual/guide_smartcheck.htm
Post the resulting log.

You're not missing any volumes? DiskPatch finds more volumes than there are in the current partition table.

I am not missing any volumes however the hard drive was used when I got it, could it have the remnants of some old, deleted partitions?

I would like to wipe the slate clean, if possible. After I backup all my data, is there a tool to clear out all the old junk?

The disk is showing its age. There are already some re-allocated sectors and health isn't 100% anymore.

There are no sector allocations in the queue so at this point the disk is okay. The disk may be okay to use for quite some time to come but if the disk is mission critical I would replace it.

Any disk wipe tool will completely clean the disk. Our DiskPatch also has this function on board.
